: Objective: To compare the application value of bionic double eyelid surgery with upper eyelid lift aponeurosis replacement and traditional open double eyelid surgery. Methods: 386 patients undergoing double blepharoplasty from March 2021 to April 2024 were selected and divided into 193 cases in each group by random sampling method. The control group received traditional open double eyelid operation, the observation group received upper eyelid lift aponeurosis replacement bionic double eyelid operation. The differences in postoperative complication rates between the two groups were compared, as well as the differences in double eyelid width and eyelid cleft height before and after surgery between the two groups. The wound healing of the patients was assessed by Vancouver scar Scale six months after surgery, and finally the satisfaction of the patients was assessed by facial plastic patient Evaluation Scale. Results: Compared with the control group, the postoperative complications were lower in the observation group (P < 0.05). Compared with the control group, the double eyelid width and eyelid cleft height were lower in the observation group (P < 0.05). The score of Vancouver scar scale was lower in the observation group than in the control group (P < 0.05). Compared with the control group, the scores of all dimensions of the facial plastic patient evaluation scale were higher in the observation group (P < 0.05). Conclusion: The bionic double eyelid surgery with upper eyelid lift aponeurosis replacement has fewer complications, and the postoperative double eyelid width is suitable, the incision healing is good, and the patients have higher aesthetic satisfaction.
