    目的:观察不同根管充填方法配合iRoot SP对牙体牙髓病患者牙周美学评分与龈沟液炎症水平的影响,为临床提供优化根管充填策略的依据。方法:选取2023年3月至2025年2月某院口腔科收治的牙体牙髓病患者60例,随机数字表法分为对照组与实验组各30例。对照组采用传统侧方加压根管充填法配合iRoot SP,实验组采用热牙胶垂直加压根管充填法配合iRoot SP。分别于治疗前及治疗后3个月评估牙周美学评分,并检测龈沟液中超敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)和白细胞介素-1β(IL-1β)水平,同时记录临床疗效。结果:治疗后,实验组牙周美学评分显著优于对照组(P<0.05),龈沟液中hs-CRP、TNF-α及IL-1β水平均明显低于对照组(P<0.05),总有效率高于对照组(P<0.05),并发症发生率方面,实验组低于对照组(P<0.05)。结论:与传统充填法相比,热牙胶垂直加压法配合iRoot SP能更显著改善牙体牙髓病患者的牙周美学效果,降低炎性因子水平,提升治疗有效率,具有更高临床应用价值。

    Abstract:

    Objective: To investigate the effects of different root canal filling techniques combined with iRoot SP on the periodontal aesthetic score and inflammatory marker levels in gingival crevicular fluid (GCF) of patients with endodontic-periodontal diseases, and to provide clinical evidence for optimizing root canal filling strategies.Methods: A total of 60 patients with endodontic-periodontal diseases treated in the Department of Stomatology of a certain hospital from March 2023 to February 2025 were randomly assigned to a control group and an experimental group (30 cases each) using a random number table method. The control group received traditional lateral compaction root canal filling combined with iRoot SP, while the experimental group received warm gutta-percha vertical compaction root canal filling combined with iRoot SP. Periodontal aesthetic scores were assessed before treatment and three months after treatment. Levels of hypersensitive C-reactive protein (hs-CRP), tumor necrosis factor-alpha (TNF-α), and interleukin-1β (IL-1β) in GCF were measured. Clinical efficacy was also recorded.Results: After treatment, the periodontal aesthetic score in the experimental group was significantly better than that in the control group (P < 0.05). Levels of hs-CRP, TNF-α, and IL-1β in the GCF were significantly lower in the experimental group than in the control group (P < 0.05). The total effective rate was higher in the experimental group (P < 0.05), and the incidence of complications was lower (P < 0.05).Conclusion: Compared with the traditional filling method, the warm gutta-percha vertical compaction technique combined with iRoot SP can significantly improve the periodontal aesthetic outcome, reduce levels of inflammatory factors, enhance treatment efficacy, and has greater clinical application value.
