    目的 分析超声骨刀微创拔牙术对下颌阻生智齿患者炎症因子水平和张口受限程度的影响。方法 选取2020年1月-2023年6月南通市中医院口腔科收治的50例下颌阻生智齿患者,按照随机数字表法分为传统组、微创组,每组25例。传统组行传统拔牙术,微创组行超声骨刀微创拔牙术,比较两组炎症因子水平、张口受限程度、恢复效果、并发症发生情况。结果 微创组术后IL-6、IL-1β均低于传统组(P<0.05);微创组张口受限0度占比为44.00%,高于传统组的12.00%(P<0.05);微创组术后创面肿胀程度、VAS评分均低于传统组(P<0.05);微创组并发症发生率(4.00%)低于传统组(24.00%)(P<0.05)。结论 在下颌阻生智齿患者中应用超声骨刀微创拔牙术所引起的炎症反应较为轻微,能有效改善术后张口受限问题,缓解术后肿痛问题,且安全性较高。

    Abstract:

    Objective To analyze the effect of minimally invasive tooth extraction with ultrasonic bone scalpel on inffammatory factor levels and mouth opening limitation in patients undergoing extraction of mandibular impacted wisdom teeth. Methods A total of 50 patients with mandibular impacted wisdom teeth admitted to the Department of Stomatology, Nantong Hospital of Traditional Chinese Medicine from January 2020 to June 2023 were selected. According to the random number table method, they were divided into the traditional group and the minimally invasive group, with 25 patients in each group. The traditional group received conventional tooth extraction, and the minimally invasive group received minimally invasive tooth extraction with ultrasonic bone scalpel. The inffammatory factor levels, mouth opening limitation, recovery effect and complications were compared between the two groups. Results After surgery, the levels of IL-6 and IL-1β in the minimally invasive group were lower than those in the traditional group (P<0.05). The proportion of patients with grade 0 mouth opening limitation in the minimally invasive group was 44.00%, which was higher than 12.00% in the traditional group (P<0.05). After surgery, the wound swelling degree and VAS score in the minimally invasive group were lower than those in the traditional group (P<0.05). The incidence of complications in the minimally invasive group (4.00%) was lower than that in the traditional group (24.00%) (P<0.05). Conclusion Minimally invasive tooth extraction with ultrasonic bone scalpel induces milder inffammatory response in patients with mandibular impacted wisdom teeth. It can effectively improve postoperative mouth opening limitation, alleviate postoperative swelling and pain, and has high safety.
