    聚左旋乳酸(PLLA)作为具有重要临床价值的生物刺激剂,能够通过诱导精准可控的宿主细胞反应,特异性激活真皮层成纤维细胞的增殖与分化,促进内源性Ⅰ型及Ⅲ型胶原蛋白的合成与沉积。此过程不仅能恢复结构性容量,还能全面改善皮肤厚度、弹性和整体肤质。其临床应用已从面部年轻化成功扩展至身体轮廓塑造,相较于传统填充剂,PLLA可提供更自然、更持久的再生效果。此外,其微粒悬浮液的物理特性提高了应用安全性,可降低血管栓塞发生风险。本综述旨在系统梳理并整合PLLA在面部及身体年轻化领域的最新研究证据,分析PLLA的生物学机制、临床应用效果、安全性及发展前景,以期为临床实践提供循证指导,为该领域的进一步发展提供理论支撑。

    Abstract:

    As a bio-stimulant with important clinical value, poly-L-lactic acid (PLLA) can speciffcally activate the proliferation and differentiation of dermal ffbroblasts and promote the synthesis and deposition of endogenous type Ⅰ and type Ⅲ collagen by precisely inducing controlled host cell responses. This process can not only restore structural volume, but also comprehensively improve skin thickness, elasticity and overall skin quality. Its clinical application has been successfully expanded from facial rejuvenation to body contouring. Compared with traditional ffllers, PLLA can provide more natural and longer-lasting regenerative effects. In addition, the physical properties of its microsphere suspension enhance application safety and reduce the risk of vascular embolism. This review aims to systematically collate and integrate the latest research evidence of PLLA in the ffeld of facial and body rejuvenation, analyze its biological mechanism, clinical application effect, safety and development prospects, so as to provide evidence-based guidance for clinical practice and theoretical support for the further development of this ffeld.
